Carole Reilly, NP-C, is a board-certified Family Nurse Practitioner based in Oceanside, CA, with over nine years of clinical nursing experience. She discovered her passion for caring for older adults in 2009 while working one-on-one with a patient living with Alzheimer’s, an experience that still guides how she practices today.
Carole has deep experience providing care in patients’ homes and has a special interest in memory changes and fall prevention. Her focus is on helping patients stay safe, independent, and well at home. Her approach centers on listening, collaboration, and prevention—taking time to understand each person’s goals, support their day-to-day health, and help reduce avoidable hospital visits.
Outside of work, Carole enjoys spending time outdoors with her husband and two daughters, as well as reading, cooking, and traveling.
